Referral, Availability and Pricing

Referrals

• Self referrals are welcome for private (non-rebated or non-NDIS sessions).

• For Medicare rebates to apply, the usual conditions of needing a referral from a medical practitioner under a Mental Health Care Plan and a limit of 6-10 rebated sessions still apply.

• Referral Form

• Please note: With the need for services currently, I have not undergone the process to become a registered NDIS provider. As such,

  • I CAN provide services to 'Self-managed' or 'Plan managed' participants,

  • I am UNABLE to provide services to NDIA participants.

Availability

• I am currently (21/12/2024) reviewing my waitlists, with potential for waitlists to change however, at this stage are:

  • Functional Capacity Assessments waitlist - I am not taking new referrals at this time.

  • 1:1 Ongoing therapy - 4- 5 months.

  Office hours are Mondays, Tuesdays and Saturdays - 9am till 6pm.

• For NDIS services, where it is home or community based, alternate arrangements can be negotiated.

Pricing

• $190 p hr.

• Fees are to be paid in full on the day for the initial session. Subsequent sessions within 7 days. For NDIS, I understand that this is not feasible, as such accept payment within 7 days for all sessions.

• Bulk billing is not available.

Cancellation Fees

• If you are unable to attend your scheduled appointment, the following is the schedule for cancellation fee's:

  • 1st and 2nd cancellation within 24 hours of appt is $100.

  • Any following cancellations within 24hrs is the full fee of the session.

  • Any cancellation that falls within 4 hrs of appt is the full fee.

  • If you have booked for a 3 hour Functional Capacity Assessment, cancellation fee will be $100 per hour pro rata, eg. 3 hr = $300.

  Pricing Schedule:

• Initial Consultation @ Office - 90 minutes - $285 (Rebate $80.30)

• 1 hr Session at offices in Warragul - $190 (Rebate $80.30)

• 1 hr Session Telehealth* - $190 (Rebate $80.30)

• 1 hr Session @ home or other offsite - $190 (Rebate $101.90)

  • Travel charged @ $190 p hr + $0.97 per km for travel there and back.

General Notes

• Out of pocket costs drastically (Rebates can be 80% of cost) reduce once safety net is reached (this can be checked in your Medicare app – under ‘Safety Net’)

• *Telehealth rebates only apply in relevant areas. Eg. Warragul / Drouin / Leongatha rebates apply, however Moe, Morewell and Taralgon rebates would not apply.

• Tick Modified Monash Model and enter relevant suburb. The map will then display the MM level of the area specified. If the area is a MM level of 4 or above, telehealth can be rebated.

Medicare Rebates

• To be able to rebate I will require a referral by a medical practitioner as per usual conditions.

• Up to 10 sessions per calendar year (With the extension from referrer)

• I require all the details on the Client Intake Online form to be able to apply for rebate. (For parents bringing their children, I require your child's details as well as your own details - Medicare needs to confirm I have the right person and they will not pay rebates to minors)

• I can apply for rebates once the fee has been paid. (Either by 'Tap and Go' or once received in bank account from invoice).

Functional Assessments and Other OT Services:

• All other OT services are charged privately at a flat rate of $190 per hour.

• At this point I am not a registered NDIS provider, however I provide functional assessments to assist in NDIS funding applications.

• If a functional assessment is required, contact me for a short intake phone call to determine the extent of the assessment in context of your particular circumstances, after which I will provide a quote and we can proceed from there.

Payment Options:

Cash, 'Tap and Go' or Direct Debit are available.

Frequency and rescheduling

Frequency of appointments will be decided in collaboration with you and in accordance with your individual needs. Every effort will be made to reschedule appointments at a mutually convenient and earliest available time.
